DIVINITY CARE SOLUTIONS TERMS AND CONDITIONS
1. ACCEPTANCE OF TERMS
By accessing or using the Divinity Care Solutions (“DCS”) services, including but not limited to our website, mobile applications, and any related products and services, you agree to comply with and be bound by these Terms and Conditions. If you do not agree to these terms, please do not use our services.
2. SERVICES PROVIDED
DCS offers disability care solutions, including but not limited to assistance with daily living activities, medical support, and therapeutic services. The specific services provided will be outlined in the service agreement between DCS and the client.
3. CLIENT RESPONSIBILITIES
Clients are responsible for providing accurate and up-to-date information about their needs and preferences. It is the client’s responsibility to communicate any changes in their condition or requirements promptly. Clients are also responsible for cooperating with DCS staff to facilitate the delivery of services.
4. SERVICE AGREEMENT
A service agreement will be established between DCS and the client outlining the scope of services, schedule, fees, and any other relevant terms. Both parties must adhere to the terms specified in the service agreement.
5. FEES AND PAYMENTS
Clients agree to pay the fees outlined in the service agreement. Payment terms will be specified in the agreement, and failure to make timely payments may result in the suspension of services. DCS reserves the right to adjust fees with notice.
6. CONFIDENTIALITY
DCS respects the privacy of its clients and will maintain the confidentiality of all personal and medical information. Client information will only be disclosed with the client’s explicit consent or as required by law.
7. CANCELLATION AND TERMINATION
Either party may terminate the service agreement with notice. DCS reserves the right to terminate services immediately if there is a breach of contract or if the safety of clients or staff is compromised.
8. QUALITY OF CARE
DCS is committed to providing high-quality care services. If clients have concerns about the quality of care, they are encouraged to communicate with DCS promptly to address and resolve any issues.
9. LIABILITY
DCS will take reasonable measures to ensure the safety and well-being of clients. However, DCS is not liable for circumstances beyond its control, and clients are encouraged to obtain appropriate insurance coverage.
10. AMENDMENTS TO TERMS
DCS reserves the right to modify these Terms and Conditions at any time. Clients will be notified of any changes, and continued use of DCS services constitutes acceptance of the amended terms.
